The device was returned to olympus for evaluation, and the customer's allegation was confirmed.In addition to the faulty power supply, the evaluation findings are as follows: the top cover was corroded and there was lint/dust accumulation on the video connector pins.Additionally, it was recommended that the software update to version 4.0 and that the narrow band imaging be updated.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer initially reported to olympus that the evis exera ii video system's power button worked intermittently and would not hold or click in place to turn on.The issue was found during a diagnostic procedure.There were no reports of patient harm.During the device evaluation, the following reportable malfunction was identified: faulty power supply.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The device history record was unable to be reviewed for this device since the serial number was not provided.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.Based on the results of the investigation, it is likely that the event was caused by a power unit failure.The root cause of the power unit failure cannot be identified.Olympus will continue to monitor field performance for this device.
