Event Description
It was reported that there was a connection issue between a minicap transfer set and the patient line of a homechoice cassette.This was identified while disconnecting from peritoneal dialysis therapy.The connection issue was further described as ¿the patient line was stuck on the dark blue connector of the transfer set".The transfer set was replaced.There was no report of patient injury or medical intervention associated with this event.No additional information is available.

H10: the device was received for evaluation with a patient connector attached to the female connector.A visual inspection with the naked eye noted a separation between the female connector and main body.Functional testing including leak, clear passage, and clamp function testing were performed with no issues noted.A connection/disconnection between the female connector and the returned patient connector was performed with no issues observed.The reported connection issue was not verified.The cause of the separation between the female connector and main body was due to inadequate solvent application to the set during the manufacturing process.Should additional relevant information become available, a supplemental report will be submitted.
