Device report from depuy synthes reports an event in china as follows: it was reported that on (b)(6) 2023, during the surgery, due to the need to adjust the position of the metal plate it was noted that three screws were broken.All the pieces were removed from the patient.Another device was used to complete the surgery.No adverse consequences to the patient.No additional information could be provided.This report is for one (1) ti matrixmidface screw self-drilling 6mm-4pk.This is report 3 of 3 for complaint (b)(4).

Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.Manufacturing location: monument, manufacturing date: 22-dec-2022, part number: 04.503.226.04c, ti matrixmidface screw self-drilling 6mm, lot number: 3551p79 (non-sterile), lot quantity: (b)(4).Component part(s) reviewed: part number: 21015, tialnbri4.00 lot number: 805p447 lot quantity: (b)(4).This lot met all dimensional, visual and packaging criteria at the time of release with no issues documented during the manufacture that would contribute to this complaint condition.The product was returned to depuy synthes for evaluation.The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the head of the screw broke off.The broken fragment was not returned for evaluation.Based on the observed condition of the returned device, the investigation was able to confirm the reported event.No other problem identified.A dimensional inspection was not performed as it is not applicable to the complaint condition.The observed condition of the device was consistent with a random component failure that may have been caused by exposure to unintended forces.As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.The overall complaint was confirmed as the observed condition of the scr ø1.5 self-drill l6 tan 4u i/clip, p/n: 04.503.226.04c, lot: 3551p79 would contribute to the complained device issue.There is no indication that a design or manufacturing issue has caused the complaint condition and hence the root cause cannot be determined.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.If information is obtained that was not available for this medwatch, a follow-up medwatch will be filed as appropriate.
