Event Description
It was reported that a minicap had a connection issue; further described as ¿did not snap in place¿.This was observed when placing the cap during peritoneal dialysis therapy.When the minicap was removed, disinfectant was noted on the outside.The transfer set remained in use.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
 
Manufacturer Narrative
H4: the lot was manufactured from february 8, 2023 to february 10, 2023.H10: the actual device was received for evaluation.A visual inspection was performed; the cap and threads were inspected for any defects which would result in a connection issue.The circumference of the sample indicated a ¿short shot¿ (molding).A functional test was performed by pairing the sample with a mock female connector to test the sample threads and the result was the sample was properly secured to the connector indicating no damage to the threads.An under-water pressure test was performed where a streamof bubbles was observed.The reported condition was verified.The cause of the condition was related to the molding process during manufacturing.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
