Event Description
The customer reported to olympus that whenever you plugged the scope into the front panel of the evis exera iii xenon light source, it made a fast-beeping noise, and then the lights would flash on the front of it.The issue was found during preparation for use; there was no delay or extension to patient anesthesia.The procedure was completed using the same set of equipment.There were no reports of patient harm.
 
Manufacturer Narrative
The device was returned to olympus for evaluation, and the customer's allegation was confirmed.The device evaluation found that the screen would flash due to a faulty scope socket.The locking mechanism and scope detection slide are not functioning.This is causing the front panel to blink intermittently.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely that the reported phenomenon ¿alarm sound went off and the front panel lit up when the scope was connected¿ occurred because the video function did not work properly due to a faulty scope socket.Olympus will continue to monitor field performance for this device.
