An olympus field service engineer (fse) was dispatched to evaluate and repair the device.The fse replaced the pump heads and valves and inspected the disinfectant tank.The device was then repaired, tested and verified to the original equipment manufacturer (oem) specifications.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.

Event Description
|
The customer reported to olympus technical assistance center (tac), during reprocessing following a diagnostic procedure, the endoscope reprocessor had black debris in the circulation mesh filter.There was no harm or user injury reported due to the event.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the identity and definitive root cause of the foreign material could not be determined.It is possible that the foreign material was present in the disinfectant tank, in the mesh filter at the circulation port.Olympus will continue to monitor field performance for this device.
