The customer inquired on the proper procedure to restart the unit after the boil water alert.A support tech emailed the oer pro instruction manual to the customer and afterwards; the customer confirmed that the issue was resolved per instructions for use.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time, however, if additional information becomes available, this report will be supplemented accordingly.

The customer reported that there was a boiled water alert related to the endoscope reprocessor.There was no patient harm or user injury reported due to the event.

This report is being supplemented to provide additional information based on the approved final investigation.The device history record was unable to be reviewed for this device since the serial number was not provided.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the boiled water alert could not be determined.The instruction manual identifies the following verbiage, which may have prevented the phenomenon: ¿4.1 installation conditions: the water temperature should be 28 (82) or lower.¿ olympus will continue to monitor field performance for this device.
