The subject device has not been returned to olympus for evaluation.The investigation is ongoing; therefore, a root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported to olympus that the visera 4k uhd xenon light source malfunctioned.The issue occurred after an unspecified therapeutic procedure.When the maj-1941-light source cable was unplugged while trying to diagnose the issue, the connector on the light source broke.There was no impact to a patient or procedure due to the event.The maj-1941-light source cable was reported in the mdr with patient identifier (b)(6).
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Based on the results of the investigation, the phenomenon was not reproduced but the probable cause of the issue likely relates to maj-1941 (light source cable) damage.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Olympus will continue to monitor field performance for this device.
