A male patient underwent an aquablation procedure for symptomatic benign prostatic hyperplasia (bph).Procept biorobotics corporation became aware that during procedure, an "e22 - motorpack error" message was generated by the aquabeam robotic system.A second handpiece was used to resolved the issue, but the error persisted.As a result, the aquablation procedure was aborted and converted to transurethral resection of the prostate (turp) surgery.Additionally, it was observed that some corrosion was present on the aquabeam motorpack.There were no adverse health consequences to the patient due to this event.

The aquabeam motorpack was returned for investigation.Visual inspection revealed signs of fluid ingress in and around the connector port, which confirmed the reported event.The root cause of the reported error is fluid ingress; however, the cause of the fluid ingress is undeterminable.A review of the device history record (dhr) ab2000-b / serial number (b)(6) and aquabeam motorpack / lot number 21c01779 was conducted, which confirmed there were no non-conformances, failures, discrepancies, or missed steps during the manufacturing process that could be related to the reported event.The review indicated that the system met all design and manufacturing specificiations when released for distribution.The current user manual um0104-00 rev.G, aquabeam robotic system user manual, intl (ce), english was reviewed.Table 5 system detected errors and faults e22 - motorpack error release foot pedal and click x.1) if error persists, reconnect handpiece to motorpack.2) if error continues, replace handpiece.Submission of this report does not constitute an admission that the manufacturer's product caused or contributed to the event.
