It was reported that a saline leak occurred.The 95% stenosed target lesion was in the moderately tortuous and severely calcified distal right coronary artery.A 1.50mm rotapro was selected for use.The device was first platformed with no issue outside the body before advancing into the patient.During the procedure, the physician had difficulty passing the lesion with the burr and after four minutes, it was noted that the burr started slowing down to 130,000rpm doing the same motion.After the burr successfully was through the lesion, it was taken out successfully.However, while using dynaglide mode, the physician noticed that the shaft of the burr had blood in it and saline was leaking in the middle portion of the burr catheter.Hence, it was determined that the leak was caused by the pushing of the whole burr along the advancer knob.The procedure was completed with the same 1.5mm burr.There was no patient complication reported.
