Event Description
The customer reported to olympus that the evis exera iii xenon light source when capturing an picture, the image would stay on the screen for 2-3 seconds rather than immediately go away.The issue was found during a procedure, which was completed without delay.There were no reports of patient harm.
 
Manufacturer Narrative
The device was not returned to olympus for evaluation.While troubleshooting with olympus technical assistance center, the customer reviewed the cabling, moved comp out to video in on monitor and removed extra cables.This appeared to resolve the picture release time issue.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
The initially reported model number was incorrect.The product was changed from the clv-190 evis exera iii xenon light source to the cv-190 olympus evis exera iii video system center.This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, and since the device was not returned for evaluation, the definitive root cause of the reported issue could not be determined.The issue was resolved via troubleshooting.Olympus will continue to monitor field performance for this device.
