Event Description
The customer reported to olympus, the visera elite xenon light source had a glare on the screen, and they had to adjust the light manually on the clv-s190 to make the tissue distinguishable.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
Olympus technical support team instructed the customer to ensure the light source cable is secure, not damaged.Part number maj-1959 was provided to order a replacement light source cable.The device was not returned to olympus for evaluation.A supplemental report will be submitted if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation the specific cause of phenomenon could not be identified as the device was not returned for evaluation.However, it is possible the imaging issue was caused by a faulty cable.Olympus will continue to monitor field performance for this device.
