Event Description
An endurant iis bifurcate stent graft was intended to be implanted in the endovascular treatment of a 56mm aaa.  it was reported that during the index procedure, when checking the contralateral side gate with the "e" marker before inserting inside the main device, the "e' mark appeared distorted; the "e" marker did not match the contralateral side gate mark and blue dot, so the contralateral side gate's deployment direction could not be confirmed. not only did cannulation become more difficult when opened on the  contralateral side gate, but the shape of the aneurysm also made the stent graft patency after placement uneasy, meaning the lumen of the aneurysm was crushed and could be pushed depending on the position where the contralateral gate is deployed. it was determined to be a defective device, and a different product was used. the procedure was completed without any problems using a different product.It was confirmed the ifu was followed.Per the physician the cause was a defective product.  no additional clinical sequalae were provided and the patient is fine.
 
Manufacturer Narrative
