The device was not returned to olympus.However, the customer¿s allegation was confirmed during an evaluation of the device in the field.The inspection results found the connecting tube could not be connected, because the connector was broken by being loosened or hit with a hard object.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, it was not possible to definitively establish the root cause of the reported event.However, it is possible that the connecting tube could not be connected, for the connector was broken by being loosened or hit with hard object and the loosened connector could have possibly been that stress was applied to the connector in the loosening direction, the stress was accumulated.Olympus will continue to monitor the field performance of this device.
 
Event Description
The customer reported to olympus, the connecting tube cannot be connected to the channel connector in the reprocessing basin of the endoscope reprocessor.The device produced an e02 error and the center plug on the yellow connector component was missing.The issue was found during maintenance.There were no reports of patient or user harm associated with this event.
