Event Description
It was reported to olympus, that the cysto-nephro videoscope distal end became knotted and was difficult to remove.The issue occurred during an urological procedure.There were no reports of patient harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the approved final investigation.Sections e2 and e3 were corrected with information that was inadvertently omitted from the initial report.The device history record was unable to be reviewed for this device since the serial number was not provided.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the definitive root cause of the knotted device could not be determined.The instruction manual identifies the following verbiage, which may have prevented the phenomenon: ¿do not coil the insertion tube, universal cord, or video cable with a diameter of less than 10 cm.Equipment damage may result.Do not attempt to bend or twist the endoscope¿s insertion section with excessive force.¿ olympus will continue to monitor field performance for this device.
