The device was returned to olympus for evaluation and the customer's allegation of the intermittent b30 error was not confirmed.The allegation of corrosion and build up was confirmed.Additionally, the device evaluation found missing print labels on the front panel, the bottom chassis was rusted, the front panel was rusted, the top cover was rusted, and heavy dust was present.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

Event Description

The customer reported to olympus, the evis exera iii xenon light source intermittently had a b30 error and corrosion and buildup at the insertion site.The customer reported that she was switching between different scopes without turning the power off.During troubleshooting on the phone olympus technical assistance center (tac) explained to the customer the proper way of connecting the scope is with the power off to avoid any spike in power surge to the endoscopes.Tac also provided the part number, maj-2087, for the scope socket cleaner for the light source.The customer was also advised that pressing the esc key to acknowledge the b30 error is important to prevent the same error from being carried over to other endoscopes, as well.The customer said she would inspect the light source closely once again and will be sending it in for repair.The issue was found during preparation for use.There were no reports of patient harm.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 7 years since the subject device was manufactured.Based on the results of the legal manufacturer's investigation, a definitive root cause of the b30 error could not be identified, nor could the phenomenon be confirmed/reproduced.Olympus will continue to monitor field performance for this device.
