Event Description
It was reported that the lps calcar planer small and the lps bevel reamer insert sz 13 were stuck together and won't come apart.The v-tamp is slightly damaged from being used.No surgical delay.

Product complaint # (b)(4).Investigation summary : an analysis of the product could not be performed since a physical sample was not received for evaluation.As part of our company quality system process, all devices are manufactured, inspected, and distributed to approved specifications.Additional complaint information monitoring for potential safety signals will be conducted through complaint trending as part of the post-market surveillance.However, if the product is received at a later date, the investigation will be updated as applicable.

Product complaint # (b)(4).Investigation summary : it was reported that the lps calcar planer small and the lps bevel reamer insert sz 13 were stuck together and won't come apart.The v-tamp is slightly damaged from being used.No surgical delay.The product was returned to depuy synthes for evaluation.The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the lps calcar planer sm has signs of normal use, the device was returned with the lps bevel reamer insert 13mm stuck together via galling most likely due to internal damage on the coupling surfaces of the devices.The functional test was performed, the device cannot be disassembled.The complaint condition was able to be replicated.A dimensional inspection was not performed as it is not applicable to the complaint condition.A potential cause cannot be established with the provided information due to be a multifactorial issue the overall complaint was confirmed as the observed condition of the lps calcar planer sm [298770001/hw89636] would contribute to the complained device issue.There is no indication that a design or manufacturing issue has caused the complaint condition and hence the root cause cannot be determined.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Device history lot : the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
