The device was returned to olympus for evaluation and the customer's allegation was confirmed.The device evaluation found that there was no image, only black/grey lines and the video connector needed troubleshooting to recheck the image.The scope passed a leakage test and all switches were noted to be working.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported the image of the endoeye flex deflectable videoscope would pop up and then abruptly cut out.The event occurred during the preparation for use.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a definitive root case of the observed no image issue could not be determined, however, it is likely that the issue was the result of a faulty printed circuit board and or video connector.The event can be detected by following the instructions for use section below: -inspection of the endoscopic image.Olympus will continue to monitor field performance for this device.
