The subject device was evaluated.Device evaluation found the bending section is squashed and dents were observed.The bending rubber found cracked and the angulation noted to be out of specifications.Follow up is in progress to gather additional information regarding the reported event.Supplemental report(s) will be submitted should any relevant new information is available and or received.Investigation is ongoing.This report will be supplemented accordingly following investigation.
 
Event Description
It was reported that the patient bit down on distal tip of the scope by accident when the bit guard came off.No known harm due to this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the event was unable to be identified.It is likely that the "bite guard" came off the patient's mouth for some reason during preparation before use.Prevention measures are described in operation manual: 4.1 insertion: insertion of the endoscope.Olympus will continue to monitor field performance for this device.
