Event Description
Philips received a complaint on the v60 ventilator indicating that there was a master alarm failure.The issue was identified outside of clinical use and no reports of user harm or injury.The customer informed the authorized service provider (asp) that they observed the message display on the monitor.The asp recommended to the customer to replace the speaker assembly.This investigation is ongoing.

Additional information received where it was reported that parts were ordered, and the device was repaired.It confirmed that the replacement parts resolved the reported issue.The investigation concludes that no further action is required at this time.If additional information is received the complaint file will be reopened.

H10: additional information received from the authorized service provider (asp) that clarified that it was actually the central processing unit (cpu) printed circuit board assembly (pcba) which was replaced, not the speaker assembly.The asp also stated that the device issue occurred during testing of the device.The investigation concludes that no further action is required at this time.If additional information is received the complaint file will be reopened.
