Event Description
It was reported that during a watchman atrial fibrillation (a fib) procedure, a versacross connect kit was selected for use.The mechanical j-wire was very difficult to remove from dilator after reaching right atrium. Therefore, the guidewire and the dilator were removed together. When the guidewire was removed, it was noted that its coils were loose and bunching up at the opening of dilator. The mechanical guidewire remained in one piece and was not stretchable. No patient complications occurred. The procedure was completed successfully. The device is not expected to be returned for analysis as it was disposed. No other issues were noted.
