Event Description
The customer reported to olympus, the evis exera iii xenon light source sometimes displayed e103.The error occurred while turning on the light source.By switching the device off and on again, the error did not occur again.The issue was found during preparation for use.No procedure or patient was involved.The customer will change the 3rd party xenon lamp and observe the device after the xenon lamp change.
 
Manufacturer Narrative
The device is not expected to be returned for evaluation.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over eleven (11) years since the subject device was manufactured.A definitive root cause was not identified.Based on the available information, the legal manufacturer was unable to determine the probable cause since the device was not returned for evaluation.Although the device was not returned, olympus was informed the customer had installed a non-olympus lamp.The event can be prevented by following the instructions for use which state: warning, non-olympus equipment can cause instability of the automatic brightness adjustment.Olympus will continue to monitor field performance for this device.
