The device was evaluated by olympus.The evaluation found that the allegation was confirmed due to a power switch failure.Additional issues were identified during the device evaluation: the front panel mouth was cracked; the top cover was rusted; the bottom chassis and front panel chassis were rusted; the main power switch was damaged; the bad scope socket was damaged; the locking mechanism was not protecting the pins; and the olympus lamp life meter was reading over 500+ hours; the lamp life was expired.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
The customer reported to olympus that the power switch on the evis exera iii xenon light source was broken.There were no reports of patient harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The device was returned to olympus for evaluation and the customer's allegation was confirmed.However, based on the results of the investigation, a root cause could not be identified.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 9 years since the subject device was manufactured.Olympus will continue to monitor field performance for this device.
