The device was returned to olympus for evaluation, and the customer¿s allegation was confirmed.The evaluation found the following: cosmetic damage (cracked front panel; front panel is slightly discolored), small amount of fluid invasion on the scope socket's connection pins and on the chassis.The device has an updated power switch and the lamp was at 50 or more hours.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus that the evis exera iii xenon light source was not working and had water inside it.The issue occurred during reprocessing after a diagnostic procedure.There were no reports of patient harm.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The device was returned to olympus for evaluation and the customer's allegation was confirmed.Based on the results of the investigation, a root cause and occurrence cause of the phenomena regarding water invasion could not be identified.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.The cause of the reported events cannot be conclusively determined.Olympus will continue to monitor field performance for this device.
