An olympus field service engineer (fse) was on site to evaluate and repair the device.The fse was able to resolve the issue of flickering video image from clv-190 by replacing power cord on steris esu unit that was located on same cart.The issue did not reoccur after the power cord was replaced.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
An olympus field service engineer reported on behalf of the customer, the evis exera iii xenon light source had a video signal interruption whenever the steris esu pm the cart would be fired.There was no harm or user injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The device was not returned to olympus for evaluation.Based on the results of the investigation, it was found that the phenomenon occurred due to the collapse of eus power cord.The field service engineer (fse) was able to resolve the issue of flickering video image by replacing power cord on steris esu unit.The issue did not reoccur after power cord was replaced.No further action is necessary.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 4 years since the subject device was manufactured.Olympus will continue to monitor field performance for this device.
