Event Description
It was reported that during a liver resection, the rubber part came off.The fallen rubber part was retrieved.Another device was used to complete the case.There were no adverse consequences to the patient.No further information is available.

(b)(4).Date sent: 10/5/2023.D4: batch # x96d2f.Additional information was requested and the following was obtained: "there was no change in the procedure (e.G.Additional port hole, additional wound ) when retrieving the fallen part.The patient is stable after the operation.".Investigation summary : the product was returned to ethicon for evaluation.Visual inspection was conducted on the returned device.Visual analysis of the returned sample determined that the b12srt device was received with the duckbill out of position and present.In addition, the duckbill was returned inside a plastic bag.Due to the condition of the retuned device had no leak tested could be performed to evaluate the reported incident.No conclusion could be reached as to what might have caused the duckbill out of position.As part of ethicon¿s quality process all devices are manufactured, inspected, and released to approved specifications.A manufacturing record evaluation was performed for the finished device batch and lot number, and no non-conformances were identified.
