Event Description
The customer reported that there was a burning smell.The reported issue was observed during an unspecified diagnostic procedure.The intended procedure was completed using the same device.There was no report of patient or user injury due to the event.
 
Manufacturer Narrative
The subject device was returned to a distributor for inspection.It was observed that the voltage stabilizer was not recommended.A stabilizer in poor condition was being used in addition to being below the recommended minimum power (500w).The voltage stabilizer will be replaced.The investigation is ongoing; therefore, a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Updated fields: h6 and h10.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 15 years since the subject device was manufactured.Based on the results of the investigation, it is likely that the burning smell was caused by a malfunction of the voltage stabilizer.Olympus will continue to monitor field performance for this device.
