Event Description
It was reported through the litigation process that a vena cava filter was placed for a patient with an unknown indication.Approximately six years and seven months post filter deployment, it was alleged that the filter had detached.The device has not been removed and there were no reported attempts made to retrieve the filter.There was no reported patient injury.

H10: as the lot number for the device was provided, a review of the device history records is currently being performed.The device has not been returned to the manufacturer for evaluation.The investigation of the reported event is currently underway.H10: d4 (expiry date: 09/2013).H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.

Manufacturer Narrative
H10: manufacturing review: a manufacturing review was not required as this is the only complaint reported to date for this product and lot.Investigation summary: the device was not returned for evaluation.Medical records were provided and reviewed.Approximately six years and seven months post filter deployment, the patient had low back pain, x-ray of lumbar spine showed the filter with fracture of at least two and possibly three of its legs.Impression given that these fractured legs appeared to have mildly migrated a short distance away from the main portion of the filter; however, their exact location was indeterminate.Around one year, three months and three weeks later, x-ray of the upper gastrointestinal tract showed the filter with the tip at the l1-2 level.There were at least two fractured struts/legs of the filter noted.Therefore, the investigation is confirmed for filter limb detachment.Based upon the available information, the definitive root cause is unknown.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.H10: d4 (expiration date: 09/2013), g3, h6 (method).H11: h6 (result, conclusion).H11: section a through f ¿ the information provided by bd represents all the known information at this time.Despite good faith efforts to obtain additional information, the complainant/reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.H3 other text : device not returned.
