Event Description
"unable to deploy the leg.When rotating the gray knob the lead screw was coming back normally but the outer sheath was not retracting.The two components appeared to be disconnected.When the lever was exposed at the back end of the black stationary grip it appeared to be broken as per figure attached.When the leg was prepared, external box had no sign of damage and no maneuver were done that could have affect the integrity of the device." patient outcome - "no issue for the patient, procedure completed with back up device.".
