Event Description
Philips received a complaint by the customer on the v60 indicating that the device is displaying proximal pressure line disconnect alarm message.It is unknown if the unit was in patient use at the time of the reported issue.No patient or user harm was reported.The manufacturer's field service engineer (fse) was dispatched to the site and confirmed the reported problem.The fse checked logs and found error message that the proximity tubing was off.Removed cover and found tubing disconnected and observed tubing to be enlarged at the connection point.The fse cut the bitter end off and reattached and noted tubing is much more secure.The device passed required performance verification tests per philips standards and was returned to service.The investigation concludes that no further action is required at this time.If additional information is received the complaint file will be reopened.

It was reported there was no patient involvement at the time the issue was discovered as it was confirmed the issue was observed during set-up.
