|
Medtronic received information that during use of a dlp high flow coronary artery ostial cannula, it was reported that after perfusion with cardioplegia, the ostial the tip of the cannula came off. the device was replaced.There was no adverse patient effect associated with this event.Medtronic received additional information that the tip was not obstructed.The tip did detach from the cannula body.The cannula was not heated or cooled prior to use.Medtronic received additional information that the tip was recovered.Medtronic received additional information that the same insertion site was used when they replaced the cannula, it is technically not an insertion site but to hold onto the ostium.There was no intervention required.Medtronic received additional information that there was an issue with a second dlp high flow coronary artery ostial cannula during the procedure.
