Event Description
The customer reported to olympus that the evis exera iii xenon light source power button was broken and right where the light source connects the bottom protector that comes out was broken.The event occurred during reprocessing.There was no patient harm associated with the event.

To date, the device has not been returned to olympus for evaluation.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Corrected field: h4 additional information added to field d8, d9, e2, e3, g2 (added health professional), h3, h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.The device was returned to olympus for inspection, and the customer's allegation was confirmed.In addition, the following non-reportable malfunctions were found during the device evaluation: damaged front panel, faulty scope socket (locking mechanism loose), screws were detached, broken, loose, and damaged, and also a non-olympus lamp was found.Based on the results of the investigation, faulty power switch was confirmed during inspection.Moreover, the front panel near the power switch was found damaged.Thus, it was surmised that it was broken due to an external impact.Olympus will continue to monitor field performance for this device.
