|
Medtronic received information that during the implant of this transcatheter bioprosthetic valve, the physician attempted to position the valve three times.During the last attempt, a "crack" was seen in the delivery catheter system (dcs) and the capsule could not be closed completely.Per the physician, the dcs was likely under a lot of tension and the patient's aortic arch was quite narrow.The valve was withdrawn and opened outside the body.A small particle (approximately 0.3mm) was found.The physician used a new valve and new dcs.The implantation was completed successfully.No adverse patient effects were reported.

Additional information was received that the part of the delivery catheter system (dcs) that "cracked" was the handle.Nothing was "protruding" from the "crack".The particle that was observed on the valve may have been a part of the valve.
