The device was returned to olympus for inspection, and the customer's reported issue ¿front panel is broken¿ was confirmed.The following findings were also noted during device evaluation: due to expired life expectancy of lamp, the light is poor, the front panel is cracked, there is a buckle on top cover, and the top cover is bent by a blow.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
The customer reported that the demo device visera elite xenon light source front panel is broken.Upon inspection and evaluation, the output connector is broken and cannot be connected.Due to a gap between the scope sockets, the scope cannot be attached.Fiber optic cable is not detected.There was no report of patient harm or user injury associated with this event.This medical device report (mdr) is being submitted to capture the reportable malfunctions found during incoming inspection and evaluation.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over ten (10) years since the subject device was manufactured.Based on the results of the investigation, the root cause could not be identified; however, it is likely that the gap between the output connector and the scope socket led to the malfunction.Olympus will continue to monitor field performance for this device.
