|
According to the reporter, during a laparoscopic totally extraperitoneal (tep) hernioplasty, the urologist tried to line the venous vessel with clip applier, but the clip applier did not line the vessel (clip did not hold tightly to the vessel and detached).The inner and outer parts of the clip separated.The device was replaced to resolve the issue.There was no patient injury.
