(b)(4).D10: cat# 163669 lot# j7384314 32mm mod head cocr std neck.G2: foreign ¿ (b)(6).The customer has indicated that the product is in process of being returned to zimmer biomet for investigation.Once the investigation has been completed, a follow-up mdr will be submitted.
 
Event Description
It was reported by the surgeon that during an initial hip procedure, the ring of the cup was bent so the cup could not be inserted into the liner.New products were used to complete the surgery.There was no known impact or consequences to the patient.Attempts have been made and no further information has been provided.
 
Manufacturer Narrative
(b)(4).This follow-up report is being submitted to relay additional information.The following sections were updated/corrected: updated: b4; b5; d4; d9; g3; h2; h3; h4; h6.Visual examination of the returned product identified the locking ring was bent inside of the shell.The chamfer was returned in the correct position.The liner shows an indentation on the outer radius.The width and thickness of the locking ring were checked per the print and found within conformance.It is unknown if the deformation occurred during or prior to the assembly attempt.Complaint confirmed based on evaluation of the returned product.Dhr was reviewed and no discrepancies related to the reported event were found.A definitive root cause cannot be determined.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
