To date, the device has not been returned to olympus for evaluation but is expected.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, the evis exera ii video system center front panel buttons were not working and the screen could not be properly white balanced.The issue was found during preparation for use.There were no reports of patient harm.
 
Manufacturer Narrative
This report is being supplemented to provide additional information, based on the legal manufacturer's final investigation and device evaluation.The device was returned to olympus for inspection.And the customer's complaint was confirmed.The front panel buttons do not work.In addition, the following non-reportable malfunctions were found, during the device evaluation: front panel lights not working and corrosion between front panel and controller board.A review of the device history record found no deviations, that could have caused or contributed to the reported issue.Based on the results of the investigation, it was confirmed, the front panel lights are faulty.The cause of the faulty front panel lights was attributed to corrosion on the controller board.Olympus will continue to monitor field performance for this device.
