The device was returned to olympus for evaluation and the customer's allegation was confirmed.The device evaluation found that the device had water leaking down into the machine causing internal corrosion, corrosion inside the air pump tubing and inside and under the scope socket tubing, worn-out socket slider switch which caused intermittent use of high intensity mode, power switch that was stuck intermittently, the rear panel rusted, the front panel opening was damaged, and the top cover and the bottom and front chassis rusted, and that the lamp life meter showed more than 300+ hours.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, the evis exera iii xenon light source had water leaking down into the device and causing possible corrosion.The issue was found during an unspecified procedure.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over eight (8) years since the subject device was manufactured.A definitive root cause could not be determined.Olympus will continue to monitor field performance for this device.
