It was reported to boston scientific corporation that a spyscope ds ii access & delivery catheter was used in the bile duct during an endoscopic retrograde cholangiopancreatography (ercp) procedure performed for the treatment of bile duct stricture on (b)(6) 2023.During the procedure, the physician decided to used spyglass after performing intraductal radiofrequency ablation to see what the tissue looked like post-ablation.The physician felt resistance during advancing the spyscope ds ii down the working channel of the duodenoscope, and they saw on the spy screen what appeared to be a metal filament at the tip of the spyscope.They withdrew both scope from the patient, and able to identify the metal filament; however, it was unclear where the metal was coming from.Reportedly, there were no problems passing any device down the duodenoscope prior using the spyscope ds ii.The procedure was completed with another of the same device.There were no patient complications reported as a result after this event.
