Event Description
It was reported that sometime post a port placement, the septum that divides the dual lumen was allegedly damaged.There was no reported patient injury.
 
Manufacturer Narrative
H10: manufacturing review: the device history records have been reviewed and this lot met all release criteria.There was nothing found to indicate there was a manufacturing related cause for this event.Investigation summary: one powerport duo mri implantable port attached to a catheter was returned for evaluation.Gross visual, microscopic and functional evaluations were performed.The t-side port septum was noted to be partially dislodged from the port body.Upon infusion of the t-side port septa, a leak from the dislodged area was observed while no water exited the distal end of the attached catheter, and after performing mandrel test, upon infusion, a leak from the dislodged port septa was observed while water with small amounts of thrombus looking material exited the distal end of the attached catheter.Aspiration was attempted and was unsuccessful.Therefore, the investigation is confirmed for material protrusion issue.A definitive root cause could not be determined based upon the provided information.Labeling review: as the reported event did not allege a labeling or use related issue, a labeling review is not required.H10: d4 (expiry date: 08/2023).H11: section a through f - the information provided by bd represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to bd.H3 other text : see h10.
