Event Description
The customer reported to olympus, parts fell off from the distal end of the uretero-reno fiberscope.The issue was found during reprocessing.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the customer's allegation of ¿parts fell off from the distal end¿ was confirmed.The device evaluation found cracked bending section cover glue and exposed threads.Additionally, there was a leak at the plastic distal end cover due to dents and burn on the plastic distal end cover.There was 1 broken fiber and jagged mask and the eyepiece body had discolored cover glass.The investigation is ongoing and follow-up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the parts falling off from the distal end was irregular stress may have been applied to the bending section cover glue.The event can be detected/prevented by following the instructions for use which state:.
