Event Description
An olympus onsite support specialist reported on behalf of the customer that the bending rubber damaged-metal protruding on the uretero-reno videoscope.The issue was found during preparation before use inspection for an unspecified therapeutic procedure.No patient was involved during the reported issue.The intended procedure was completed with another similar device without delay.Patient condition had no negative reports following the procedure, no documentation had been noted for any such occurrences.There were no reports of patient or user harm associated with this event.

The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The device was returned to olympus for inspection, and the customer's complaint was confirmed.Device evaluation found scope bending section cover is damaged/leaking from cracked glue.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely that the event occurred due to the following: during device handling by the user, physical stress was applied to bending section, which led to occurrence of a-rubber (bending section cover) cut.However, metal did not protrude out of a-rubber.The event can be prevented by following the instructions for use (ifu) which state: "-inspection of the endoscope -do not strike, hit, or drop the endoscope¿s distal end, insertion tube, bending section, control section, universal cord, video connector, or light guide connector.Also, do not bend, pull, or twist the endoscope¿s distal end, insertion tube, bending section, control section, universal cord, video connector, or light guide connector with excessive force.The endoscope may be damaged and could cause patient injury, burns, bleeding, and/or perforations.It could also cause parts of the endoscope to fall off inside the patient." olympus will continue to monitor field performance for this device.
