Event Description
|
The customer reported an intermittent image, and the evis exera iii xenon light source lens was damaged.The issue was observed during preparation for use.There was no patient or user harm reported due to the event.

Manufacturer Narrative
|
The subject device was returned to an olympus service center for evaluation.The inspection and testing found the turret lens filter was broken/cracked but could not confirm the customer¿s reported issue.However, the device evaluation found the slider switch on the scope connector socket was worn, causing intermittent high intensity mode.Additionally, corrosion was observed inside the scope socket tubing.The front panel was damaged, and the mouth was cracked, the chassis was rusted in the front and bottom side.The lamp light intensity measured out of range, and burn marks were observed on the lamp lens.There was also a non-olympus lamp present.Scratches were observed on the top cover and lamp door.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the legal manufacturer's investigation, a definitive root cause of the intermittent image could not be identified, nor could the phenomenon be confirmed/duplicated.Also, based on the results of the legal manufacturer's investigation, a definitive root cause of the light exploding inside could not be identified.However, it¿s likely the cause is related to the device being damaged.Olympus will continue to monitor field performance for this device.
