Event Description
A physician reported that during a cataract/vitrectomy combination surgery the surgeon has used anterior vitrectom with an ophthalmic operating console the used vitrectom turned out to be incompatible with the system.The issue caused delay in surgery necessitating its interruption after suturing the incisions.The patient was given with topical anesthesia and due to the delay in surgery and patient experienced ocular hypertonia , pain and nausea.Surgery was resumed under general anesthetic and was completed using another ophthalmic system and the patient condition is in stable now.

Investigation including root cause analysis is in progress.A supplemental mdr will be filed as necessary in accordance with 21 cfr 803.56 when additional reportable information becomes available.The manufacturer internal reference number is: (b)(4).
