Event Description
Philips received a complaint from the customer reporting a patient disconnect condition that resulted in low tidal volume on the v60 ventilator.The device was in clinical use.The user replaced the patient nasal catheter and exchanged the v60 for another ventilator to continue therapy.The patient reported chest tightness at the time of the event.No other clinical information or medical intervention was reported.Additional information has been requested regarding the reported event.
 
Manufacturer Narrative
A philips authorized service provider (asp) confirmed with the customer there was no injury or other harm resulting from this event.The customer biomedical engineering department resolved this issue by replacing the gas delivery system (gds).The provision of additional details requested regarding the clinical impact and the correction were declined by the customer.The device was operational and returned to service after repairs were completed.The investigation concludes that no further action is required at this time.If additional information is received the complaint file will be reopened.
