Event Description
|
It was reported to olympus that the shockpulse lithotripsy transducer during a therapeutic percutaneous nephrolithotomy (pcnl) procedure there was a physical defect of the connecting piece, and the end was cracked.The procedure was completed with the same device.There was no patient harm associated with the event.The device was evaluated, and it was found that the device suction port end was broken.This mdr (medical device report) is being submitted to capture the reportable malfunction found during the device evaluation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and to correct d4 udi.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely the suction port damage occurred accidentally due to improper device handling.However, the root cause of the complaint could not be conclusively determined.The event can be prevented by following the instructions for use which state: - "this product is a precision device; handle it with care.Avoid rough or violent handling, which may cause equipment damage." - "damage (mechanical and electrical) may result if the unit is dropped or struck against another object." olympus will continue to monitor field performance for this device.
