The device was returned as part of the asset return process due to deformation of video connector; water tightness is lost, adhesive on bending section cover has a chip, video connector case has a crack, due to wear of angle wire, bending angle in up direction does not meet specifications, venting connector of light guide connector is deformed.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided.

A user facility returned the olympus asset, endoeye flex deflectable videoscope, to the olympus service center.Upon inspection and testing of the returned device, it was observed that ¿due to damage on ccd unit, image noise occurs, and the image cannot be seen.¿ this report is being submitted for the event found during evaluation.There was no patient involvement.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and to correct h6: problem code (1036-signal artifact noise should not be selected on the initial medwch).A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause could not be conclusively determined.It is likely the reported event occurred due to a damaged image sensor unit or broken mounting components (chip, capacitor, ect.) on the electrical board caused by stress from external factors or handling.The event can be detected by handling the device in accordance with the following instructions for use( ifu): "[inspection of the endoscope] confirm that the wli and nbi endoscopic images are normal.1.Before inspection, wipe the objective lens using clean lint-free cloths moistened with saline solution or sterilized water.2.Observe the palm of your hand in the wli and nbi endoscopic images.3.Confirm that light is output from the endoscope¿s distal end.4.Adjust the brightness level as appropriate.5.Confirm that the wli and nbi endoscopic images are free from noise, blur, fog, or other irregularities.6.Turn the angulation control levers slowly in each direction until it stops.7.Confirm that the wli and nbi endoscopic images do not momentarily disappear or display any other irregularities." olympus will continue to monitor field performance for this device.
