Event Description
It was reported to philips that the heartstart xl device does not read the ecg with disposable plates.There was no reported patient impact or injury.
 
Manufacturer Narrative
This report is based on information provided by philips subcontractor technician and has been investigated by the philips complaint handling team.Philips received a complaint on the heartstart xl defibrillator indicating that the device does not detect the ecg trace with disposable plates.There was no patient harm.Based on the information available, when the device was checked at the customer site, the root cause of the reported issue was not identified as there was no fault found.The customer was advised to replace the device ac power component as a preventive measure.The device is working fine as per manufacturer's specifications.If additional information becomes available to indicate further evaluation is warranted, this record will be reopened for reassessment.
 
Event Description
It was reported to philips that the heartstart xl device does not read the ecg with disposable plates.Although no direct adverse event to the patient or user was reported, we are considering this to be a serious injury due to a serious deterioration in the state of health of the patient because life-saving therapy/treatment may have been interrupted/delayed.
