The device was returned to olympus for evaluation.The device evaluation also found that the rear panel was missing characters.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, the insufflator kept alarming when the air flow was stopped.The issue was found during an unspecified event.The device was returned for evaluation.During the device evaluation, the following reportable malfunction was found: insufflator kept alarming when air flow was stopped due to faulty printed circuit board.There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and feedback from the field.It was confirmed that air supply stop occurred at all of the following: after air is supplied and the inside of the body cavity (model at the time of inspection) becomes overpressure.Before starting up and starting air supply immediately after the start of air supply or during air supply (assuming the period until the measured value reaches the set value).A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported event is unable to be determined.However, the cause of the event is likely due to failure of the pressure sensor (cr) board.Olympus will continue to monitor field performance for this device.
