|
Valiant captivia stent graft (vamc3834c150tj) was intended to be implanted during a two-part endovascular treatment of an 53mm thoracic aortic aneurysm.The patient had undergone a y graft replacement approximately 20 years previously , where the blood vessel diameter measured 8mm. it was determined that there would be no issues with passing through the delivery system so the procedure went ahead as usual.It was reported during the index procedure, when the peripheral device vamc3834c150tj was inserted in the right main, it did not advance and became stuck in the peripheral anastomotic region of the y graft.The graft was withdrawn and was inserted in the left side which was newly cut down, the device advanced further than the right but stopped at the leg of the y graft.Multiple attempts were made to advance the device, but it was ultimately decided to withdraw the delivery system.This decision was based on concerns that the device might become irretrievable if advanced further in the first run or that the second run might not be successful. it was noted that non-mdt devices , which are lower profile devices than valiant captiva, were not available as back ups at the time.Per the physician the cause of the delivery issue was related to the fact that 20 years that passed since the placement of the y graft which made this procedure more difficult.No additional clinical sequelae were provided, and the patient will be monitored.
